外周血γδ T细胞及其亚群在炎症性肠病患者中变化的初步临床研究

背景:炎症性肠病(IBD)主要包括克罗恩病(CD)和溃疡性结肠炎(UC),是一组慢性复发性自身免疫病。研究表明γδ T细胞在IBD的发生、发展中扮演重要角色。目的:初步探究IBD患者外周血单个核细胞(PBMCs)中γδ T细胞及其亚群的比例变化和临床意义。方法:纳入2022年11月—2024年5月就诊于苏州大学附属第一医院和南京医科大学附属苏州医院(北区)的活动期IBD患者100例,其中CD 66例,UC 34例。同期30名健康体检者作为对照组。采用流式细胞术检测PBMCs中的γδ T细胞及其亚群[白细胞介素(IL)-17+ γδ T细胞、干扰素(IFN)-γ+ γδ T细胞]比例。结果:与对照组相比,活动期IBD患者PBMCs中γδ T细胞、IL-17+ γδ T细胞比例显著增加(P均<0.05),IFN-γ+ γδ T细胞比例则无明显变化(P>0.05)。轻、中、重度活动期IBD患者γδ T细胞、IL-17+ γδ T细胞比例总体差异显著(P均<0.05)。在CD和UC患者中,γδ T细胞、IL-17+ γδ T细胞比例分别与克罗恩病活动指数评分和改良Mayo评分呈显著正相关(CD: rs=0.344, 0.529, P均<0.01; UC: rs=0.540, 0.618, P均<0.01)。结论:IBD患者PBMCs中γδ T细胞和IL-17+ γδ T细胞比例明显升高,并与疾病活动性和严重程度相关,提示γδ T细胞及其亚群可能在IBD的免疫病理过程中发挥重要作用。

Preliminary Clinical Study on Alterations of Peripheral Blood γδ T Cells and Their Subsets in Patients With Inflammatory Bowel Disease

Background: Inflammatory bowel disease (IBD), which mainly encompasses Crohn′s disease (CD) and ulcerative colitis (UC), is a chronic and recurrent autoimmune disorder. Studies have demonstrated that γδ T cells play a significant role in the occurrence and development of IBD. Aims: To preliminarily investigate the alterations and clinical significance of γδ T cells and their subsets in peripheral blood mononuclear cells (PBMCs) of IBD patients. Methods: A total of 100 patients with active IBD who were treated at the First Affiliated Hospital of Soochow University and the Affiliated Suzhou Hospital of Nanjing Medical University (North Branch) from November 2022 to May 2024 were enrolled, including 66 patients with CD and 34 with UC. Thirty healthy volunteers undergoing routine physical examination during the same period were recruited as the control group. The proportions of γδ T cells and their subsets [interleukin (IL)-17+ γδ T cells and interferon (IFN)-γ+ γδ T cells] in PBMCs were measured using flow cytometry. Results: Compared with the control group, the proportions of γδ T cells and IL-17+ γδ T cells in PBMCs were significantly increased in active IBD patients (all P<0.05), while no significant difference was observed in IFN-γ+ γδ T cells (P>0.05). There were significant differences in the proportions of γδ T cells and IL-17+ γδ T cells among patients with mild, moderate, and severe active IBD (all P<0.05). The proportions of γδ T cells and IL-17+ γδ T cells were positively correlated with the Crohn′s disease activity index score and modified Mayo score in CD and UC patients, respectively (CD: rs=0.344, 0.529, all P<0.01; UC: rs=0.540, 0.618, all P<0.01). Conclusions: The proportions of γδ T cells and IL-17+ γδ T cells in PBMCs are significantly increased in IBD patients and are correlated with the disease activity and severity. These findings suggest that γδ T cells and their subsets might play an important role in immunopathogenesis of IBD.
