| 非糜烂性反流病(NERD)是胃食管反流病(GERD)的常见亚型,其临床表现以反复烧心、反酸为特征,但内镜检查缺乏明显食管黏膜损伤。NERD的发病机制复杂且具有异质性,传统的胃酸反流学说难以全面解释其症状的发生和病程的持续性。近年来,随着高通量测序技术和微生物组学的发展,越来越多的研究发现上消化道菌群可能在NERD的发生和进展中发挥重要作用。本文在系统梳理NERD的临床特征和主要发病机制的基础上,重点阐述上消化道菌群与NERD之间的潜在关联,并进一步探讨基于菌群调控的干预策略和未来研究方向。深入理解上消化道微生态与NERD之间的相互作用,有望为该病的预防、诊断和个体化治疗提供新的思路和研究方向。 |

Mechanistic Association and Clinical Implications of Upper Gastrointestinal Microbiota in Non-erosive Reflux Disease

| Non-erosive reflux disease (NERD) is a common subtype of gastroesophageal reflux disease (GERD), which is clinically characterized by recurrent heartburn and acid regurgitation without obvious esophageal mucosal lesions under endoscopy. The pathogenesis of NERD is complex and heterogeneous, and the traditional theory of gastric acid reflux cannot fully explain the onset of symptoms and persistent disease course. With the advances in high-throughput sequencing technology and microbiomics in recent years, accumulating studies have demonstrated that upper gastrointestinal microbiota may play a vital role in the occurrence and progression of NERD. Based on a systematic review of the clinical features and major pathogenesis of NERD, this article focused on elaborating the potential association between upper gastrointestinal microbiota and NERD, and microbiota-based intervention strategies and future research directions were further discussed. An in-depth understanding of the interaction between upper gastrointestinal microecology and NERD was expected to provide novel insights and research directions for the prevention, diagnosis and individualized treatment of this disease. |
