原发性胆汁性胆管炎药物治疗研究进展

| 原发性胆汁性胆管炎(PBC)是一种慢性进行性胆汁淤积性肝病,长期以来熊去氧胆酸(UDCA)是唯一被广泛采用的治疗药物,但仍有约40%的患者对UDCA治疗无应答,亟需探索新的二线治疗药物。目前PBC的治疗主要集中于缓解胆汁淤积,针对新靶点的调节剂研究取得较大进展。基于抗原特异性调节性T细胞和自然杀伤细胞的免疫调控策略,以及嵌合抗原受体T细胞等免疫治疗方法,已在PBC治疗中初步显示出有效性。具有抗炎作用的天然药物(如小檗碱、巴马汀、表没食子儿茶素没食子酸酯等)也可能成为治疗PBC的候选药物。此外,肠道菌群与PBC之间存在密切关联,使肠道微生态调控有望成为潜在的治疗方法。本文就PBC药物治疗研究进展作一综述。 |

Advances in Pharmacotherapy of Primary Biliary Cholangitis

| Primary biliary cholangitis (PBC) is a chronic, progressive cholestatic liver disease. For many years, ursodeoxycholic acid (UDCA) has been the only widely used first-line therapeutic agent; however, approximately 40% of patients do not respond adequately to UDCA, highlighting the urgent need for novel second-line therapies. Current treatment strategies primarily focus on alleviating cholestasis, and significant progress has been made in the development of modulators targeting new pathways. Immunomodulatory approaches, such as those involving antigen-specific regulatory T cells and natural killer cells, along with immunotherapies including chimeric antigen receptor T cell (CAR-T) therapy, have shown initial promise in the treatment of PBC. Additionally, natural products with anti-inflammatory properties, such as berberine, palmatine, and epigallocatechin-3-gallate, may serve as potential candidate drugs for PBC. Furthermore, the established link between gut microbiota and PBC suggests that modulation of the intestinal microecology could represent a promising adjunctive therapeutic strategy. This article reviewed advances in the pharmacotherapy of PBC. |
